On the RookieMoms list:
You can make your own pumping bra by cutting slits in a regular bra big enough for a the pump to fit through, and sewing velcro on top to keep it closed. Yes, it's ugly as hell but when you're pumping you're going to look and feel like a dairy cow no matter what, and no one's going to be looking at you anyway. I made 2 of these out of cheap old bras and they lasted forever.
Swaddle blanket, soft carrier (not the bjorn kind where the baby hangs by his crotch - this is bad for his pelvic development. I recommend one of the Asian mei tai style and one shoulder sling), and extra laundry basket -- all this is the truth.
If you're breastfeeding you don't need a bottlewarmer.
Stroller that can push around car seat - Not if you live in NYC or somewhere else where you're on public transportation all the time. Wear that baby, don't mess with a bigass stroller on the subway stairs.
I would put the nursing pillow in the "Must Have" category and relegate the Pack n Play to the "Nice to Have" category. Takes up too much space, I never had one.

Totally agreed with mjoe on every point! Ditch that bulky stroller if you can. A mei tai, mobywrap (www.thegoodmama.com) or pouch-sling are safe and efficient ways to carry baby around.
As far as green laundry is concerned: don't forget the unbleached prefold diapers and hand-knit wool diaper covers!
